Mr. Gephart was admitted to Post Acute Specialty Hospital for continued management of respiratory failure and severe debility from prolonged hospitalization. His prior level of function was independent at home. He was minimal to moderate assist with mobility and transfers and moderate to maximal with self-care tasks along with needing oxygen with all activity.
He was highly motivated with therapy efforts and pushed himself to do more with therapy. At time of discharge he improved with all ADLS and mobility to be able to return home. He also did not have to use oxygen during activity. We are happy to report that Mr. Gephart has been discharged to his home and thankful for the help from his therapists!
