Some patients may be experiencing multi-system complications while others may be coming from the ICU. We also welcome you to contact us directly even without a referral from your healthcare provider. 

Regardless of how you come to us for care, you will work with a patient coordinator who will handle everything from evaluating your needs to helping you transition into post-acute care.
