Our Admissions Process

Most of our patients are referred to us by their healthcare provider. Typically, our patients are dealing with medically and physically compromised conditions that require hospitalization. Others are admitted from the ICU to receive the acute care they need before they transition into their next level of care or return home.

We also accept patients without a referral. If you are a prospective patient or family member, we encourage you to reach out to us about our long-term acute care programs.

Regardless of how you enter our care, a dedicated patient coordinator will be by your side throughout each step of the process — starting with your initial evaluation until your transition into post-acute care.
